Diabetic products: what changes in them besides the price

The label "for diabetic nutrition" describes one property of a product — it contains fewer simple sugars than its standard counterpart. The regulation says nothing about fat, calorie content, or serving size in this definition, so diabetic waffles remain waffles by all metrics except one.

What the regulations define as a diabetic product

The definition is set out in TR TS 027/2012: diabetic food products are products "in which the content of easily digestible carbohydrates (monosaccharides — glucose, fructose, galactose, and disaccharides — sucrose, lactose) is absent or reduced relative to their content in similar food products and/or the carbohydrate composition has been modified."

Three things follow from this wording. The comparison is made against a standard equivalent, not against a specific regulatory standard. The composition can be changed in two ways: by removing sugar or by rearranging the carbohydrates. Fats are not mentioned in the definition, so no one is required to change them.

This leads to the typical result on the shelf: cookies where sucrose is replaced by fructose or maltitol, while the margarine, flour, and calorie content remain the same.

Why calorie content does not drop as expected

The energy value on the packaging is calculated not by actual measurement, but using coefficients from Annex 4 to TR TS 022/2011. These explain how much can be gained by substitution:

Let's calculate this for a cookie that contained 20 g of sugar and 22 g of fat per 100 g. Replacing sugar with fructose changes nothing: it was 4 kcal/g, and it remains 4 kcal/g. Replacing it with maltitol removes 32 kcal out of approximately 480 — this is less than 7% of the total calories. Meanwhile, the fat provides 198 kcal and remains unchanged because it has nothing to do with the word "diabetic."

Fructose: what the trials showed

The idea behind the substitution is that fructose raises blood glucose less than sucrose. In a meta-analysis of 18 controlled trials with 209 participants, replacing other carbohydrates with fructose while maintaining the same caloric intake improved long-term glucose control — the effect was equivalent to a reduction in HbA1c of approximately 0.53%.

The key phrase here is "while maintaining the same caloric intake." In the trials, fructose was not added on top of the diet but replaced other carbohydrates. Cookies with fructose eaten in addition to regular food have nothing to do with this concept.

Secondly, the fructose that a manufacturer puts into a product remains a free sugar, and the WHO recommendations on free sugar intake apply to it just as they do to regular sugar.

Sugar alcohols and the warning people rarely read

Maltitol, sorbitol, xylitol, and isomalt are only partially absorbed in the small intestine; the rest reaches the microflora of the large intestine. This causes bloating and a laxative effect in some people — this is a property of the substance, not a sign of spoiled product.

The regulations consider this effect significant enough to make a warning mandatory. Clause 12 of Part 4.4 of Article 4 of TR TS 022/2011 requires that immediately after the list of ingredients, the following statement must appear: "Contains sweetener(s). Excessive consumption may have a laxative effect."

What to check on the packaging in ten seconds

  1. Calories per 100 g. Compare it with a standard equivalent of the same type, not with expectations based on the word "diabetic."
  2. The line "carbohydrates, including sugars." It shows exactly what was reduced and what was substituted.
  3. Fats. In confectionery, they provide more than half of the calories, and replacing sugar does not affect them.
  4. The first three ingredients. Components are listed in descending order of mass, and flour and fat are usually listed before the sweetener.
  5. The warning about a laxative effect. A sign that sugar alcohols are present.

Who needs these products and who does not

For a person without diabetes, a "diabetic" product offers nothing but a higher price: sugar alcohols and fructose do not fundamentally change the nature of a confectionery product.

With diabetes, diet is part of the treatment, and decisions about it are made by the doctor together with the patient. The label on the packaging neither cancels nor replaces medical advice: it informs about the product's composition, not whether it is suitable for a specific person or in what quantity.
