Magnesium: why blood tests reveal little and what the trials have shown

Magnesium is a substance with a reputation as a universal answer: from calf cramps to anxiety and poor sleep. This reputation rests on two things that are irrelevant: magnesium is involved in hundreds of enzymatic reactions, and it is easy to purchase. Neither of these facts suggests that a supplement will change anything.

The problem with testing

The adult human body contains about 24 grams of magnesium. Roughly half is found in the bones, and most of the rest is inside cells. Less than one percent is in the serum, and the body maintains its concentration within a narrow range by drawing magnesium from its stores.

A review on the biochemistry and assessment of magnesium states the consequence of this clearly: measuring serum concentration is the most common and accessible way to assess status, but serum levels do not have a reliable correlation with either the total magnesium content in the body or the concentration in specific tissues. It does not follow from this that the test is useless. It follows that a normal result does not answer the question for which it was ordered.

Three trials on one hundred and fifty-one people

The most common promise. A systematic review with meta-analysis found only three randomized trials of oral magnesium for insomnia in the elderly — 151 participants across three countries. The combined result: sleep onset latency after the intervention was 17.4 minutes shorter than in the placebo group. Total sleep time increased by 16 minutes, but this difference is not statistically significant.

The authors rated all included studies as having a moderate or high risk of bias and the quality of evidence as low to very low, concluding that there is insufficient data for confident recommendations. Review in BMC Complementary Medicine and Therapies.

A broader review of 31 trials of various sleep supplements is also telling. A significant improvement in subjective sleep quality was found there for amino acids, melatonin, and vitamin D. Forms: how they actually differ

The difference between forms is real, but it mainly concerns two things: how much elemental magnesium is in a gram of salt and how well the salt dissolves.

None of the forms turn a weak evidence base into a strong one. The difference between them is about tolerability and bioavailability, not about whether the intervention works.

Where magnesium comes from in food

The main sources are legumes, nuts, seeds, whole grains, and green leafy vegetables. Processing grain reduces the content: when white flour is produced, the bran and germ, where most of the magnesium is, are removed. Loss through sweat during prolonged exertion in heat is also worth noting, but its scale is usually exaggerated: it is primarily sodium that is lost in sweat. More details — in the article about water and electrolytes.

Safety limits

Magnesium supplements have a predictable dose-dependent side effect — diarrhea, which also limits overdose in people with functioning kidneys. With reduced kidney function, the situation is the opposite: excretion is impaired, and magnesium can accumulate. Magnesium also interacts with a number of medications, reducing their absorption, so for those on constant therapy, the issue should be discussed with a doctor.

A weak evidence base does not mean the substance is dangerous. It means the expected effect is small and poorly predictable — much like what is described in the article about self-experimentation.
