Cheat meal and refeed: do breaks in a calorie deficit help?

Three terms describe three different things, and the confusion between them costs more than it seems. A cheat meal is a one-time deviation from the plan, a refeed is a planned increase in carbohydrates, and a diet break is a return to maintenance calories for a week or two. Research has primarily examined the latter two, and the results have proven more modest than expected.

The key difference is whether it is calculated in advance. A refeed and a diet break are part of a plan with known caloric intake; a cheat meal is by definition untracked, which is exactly why it most often wipes out a week's deficit.

The rationale provided

The logic is as follows: during a prolonged deficit, energy expenditure decreases more than can be explained by weight loss, and the level of hormones regulating appetite shifts toward hunger. A periodic increase in intake, especially of carbohydrates, is supposed to partially offset these shifts.

Some of the premises here are correct: the adaptation of expenditure during prolonged restriction is well-documented and analyzed in our article on weight loss plateaus. What is debatable is the next step—that a short increase in intake significantly reverses this adaptation.

What controlled comparisons have shown

A study involving a two-day carbohydrate refeed every week against a background of about 25% restriction reported better preservation of lean mass compared to continuous restriction. The result, however, was disputed: upon re-analysis, the difference remained only for one of the derived variables, not for lean mass as a whole.

Longer breaks have been tested separately. In a randomized study on trained women, researchers compared continuous six-week restriction with the same total amount of restriction broken up by one-week breaks: no differences were found between the groups in terms of body composition changes or resting metabolic rate.

In summary: there is little convincing data that breaks provide an advantage in body composition, and the existing data is contradictory. This does not mean the practice is useless—it means that the benefit, if it exists, does not lie in physiology.

Where the benefit actually lies

It is psychological, and this is not a dismissive assessment: breaking a diet is also not a physiological event.

The arithmetic of a cheat meal

It is worth doing the math here because intuition often fails. A deficit of 400 kcal per day yields 2,800 kcal over a week. An untracked outing—pizza, dessert, a couple of drinks—easily costs 2,000–3,000 kcal above a normal day. In other words, one cheat meal can wipe out a week's progress.

Hence the practical difference: a refeed is calculated and accounted for, while a cheat meal is not. The former is compatible with a deficit, while the latter competes with it.

How to do it if you want to

  1. Plan in advance. The date is known, the food is chosen, and the portion size is envisioned.
  2. Increase intake through carbohydrates, not fat. At the same caloric value, this restores glycogen better and has less impact on the overall total.
  3. One meal, not one day. A "cheat day" is almost always 1,500–2,000 extra kilocalories instead of 500.
  4. Track at least approximately. Even a rough estimate prevents you from reaching scales that wipe out a week's work.
