Visceral fat: why it matters and how to measure it
Two people at the same weight, with the same BMI, can carry very different metabolic risk. The difference is often not how much fat they have but where it sits. Fat packed inside the abdomen behaves differently from fat under the skin, and a tape measure picks up that difference better than a scale does.
Two kinds of fat, and a third
Subcutaneous fat lies between the skin and the muscle wall. It is the layer you can pinch, and on most people it is thickest over the hips, thighs and the backs of the arms. Visceral fat sits deeper, inside the abdominal cavity itself — packed into the omentum and the mesentery that drape over and between the intestines, and wrapped around the liver and kidneys. Because it sits behind the abdominal muscles, a large visceral depot reads from outside as a firm, rounded belly that does not gather into a fold.
There is a third kind that gets discussed less: fat stored inside the organs rather than around them, above all in the liver. This is what underlies fatty liver disease, and it is usually found by accident — on an ultrasound ordered for something else, or from liver enzymes that came back mildly raised on a routine panel.
The distinction is not cosmetic. Someone whose weight and BMI look unremarkable can still carry a substantial visceral depot, and BMI has no way of telling you so. It is a ratio of weight to height, and it cannot distinguish muscle from fat, let alone one fat depot from another.
Why visceral fat gets a separate name
Visceral fat is metabolically active in a way subcutaneous fat is not. Its cells turn lipid over faster: they respond more strongly to the hormones that release fat and less strongly to insulin's signal to hold it. The result is a steady output of free fatty acids from a depot that is harder to switch off.
Where those fatty acids go is the second half of the story. Blood leaving visceral fat drains through the portal vein straight into the liver, while blood leaving subcutaneous fat joins the general circulation and is diluted across the whole body first. So the liver sees a concentrated stream that the rest of the body does not. The consequences described in that setting are hepatic insulin resistance, more glucose released by the liver, and more triglyceride-rich particles exported into the blood — the pattern that shows up on a lipid panel as high triglycerides with low HDL.
Fat tissue is also an endocrine organ, and visceral fat is a noisier one. It secretes more inflammatory signalling molecules — interleukin-6, TNF-alpha and others — and less adiponectin, the hormone associated with better insulin sensitivity. That combination sustains the low-grade inflammation repeatedly found alongside insulin resistance and cardiovascular disease.
Worth saying plainly: the association between visceral fat and metabolic disease is robust and holds after adjusting for total body fat, but the causal chain above is a well-supported explanation rather than a closed case. A competing reading is that a large visceral depot is partly a marker — a sign that subcutaneous tissue has run out of room to store fat safely, and the overflow has gone somewhere it does less good. Both readings point the same way in practice, which is why the practical advice does not depend on settling the argument.
How it is measured properly, and why you will not do that
The reference methods are computed tomography and MRI, which image a single cross-section of the abdomen — usually at the level of the fourth or fifth lumbar vertebra — and let visceral fat area be measured directly. DEXA scanners estimate it too, from a model rather than from a direct image. All of these belong to research and clinical settings: they are expensive, they need an appointment, and in the case of CT they carry a radiation dose that nobody should accept for curiosity about their waistline.
What is left for everyday use is a tape measure. It is a proxy rather than a measurement, but it is a good one: waist circumference tracks visceral fat area closely enough that it predicts metabolic and cardiovascular risk over and above BMI, and it costs almost nothing.
Measuring your waist so the number means something
The protocol matters more than people expect, because a few centimeters of vertical position changes the reading more than a month of effort does.
Measure against bare skin or a single thin layer. Stand with your feet about 25–30 cm (10–12 in) apart and your weight even on both, arms relaxed at your sides. Find the lowest rib you can feel and the top of the hip bone on the same side, and take the midpoint between them; that is the level for the tape. Run the tape horizontally around the body at that level, parallel to the floor all the way round — check the back in a mirror, because that is where it slips. Pull it snug without denting the skin.
Take the reading at the end of a normal breath out. Do not hold your breath, and do not pull your stomach in. Measure twice and average; if the two readings differ by more than about a centimeter, measure again.
One caveat about protocols. The midpoint method above is the WHO's, and it is the one the common thresholds were built on. Other surveys measure at a different level — just above the top of the hip bone, or at the navel — and those can differ by a couple of centimeters on the same person. Pick one method and keep it. Comparing a measurement taken at the navel today against one taken at the midpoint last spring tells you nothing about your body, only about your tape position. For tracking, measure at the same time of day, ideally in the morning before eating.
What the thresholds say, and who they were drawn for
The WHO's widely quoted cut-offs mark increased risk at 94 cm (37 in) for men and 80 cm (31.5 in) for women, and substantially increased risk at 102 cm (40 in) for men and 88 cm (34.5 in) for women. Those numbers were derived mainly from European populations, chosen to correspond roughly to BMI thresholds of 25 and 30.
They are not universal, and the bodies that publish them do not claim they are. The International Diabetes Federation defines central obesity with cut-offs that vary by sex and by ancestry: 94 cm for men and 80 cm for women of European descent, but 90 cm (35.5 in) for men of South Asian, Chinese, Japanese and South or Central American descent, with 80 cm retained for women in those groups. For sub-Saharan African and Middle Eastern populations, European values are used as an interim measure because the data to set separate ones is thinner.
The reason is not arbitrary. At the same waist circumference, people of South Asian ancestry tend to carry more visceral fat and show metabolic disturbance at a lower measurement, so a single global number would over-diagnose some populations and miss others entirely. If you see one threshold quoted with no mention of sex or ancestry, it has been simplified past the point of usefulness.
Whatever cut-off applies to you, treat it as a prompt rather than a verdict. Crossing it is a reason to have blood drawn and blood pressure taken, not a diagnosis in itself.
Waist-to-height ratio: the version that needs no table
Divide your waist by your height in the same unit. The result is dimensionless, so it works identically in centimeters or inches, and the target is the same for nearly everyone: keep it below 0.5. UK national guidance recommends this measure for adults, alongside BMI, for exactly that reason — one number, no lookup, and it scales automatically with frame size.
At 175 cm (5 ft 9 in) that means keeping the waist under about 87.5 cm (34.5 in). At 160 cm (5 ft 3 in), under 80 cm (31.5 in). The ratio absorbs most of what the separate cut-off tables were doing, which is why it has become the first-line screen in several guidelines.
The "visceral fat rating" on a smart scale
Bathroom scales that report a visceral fat number do not measure visceral fat. They pass a small current through the body and measure how much the tissues resist it — fat conducts poorly, lean tissue conducts well — and then feed that impedance value into the manufacturer's own regression equation, calibrated once against a reference group scanned by a real method. Nothing in that chain looks inside the abdomen. A foot-to-foot scale in particular sends most of its current up one leg and down the other, sampling the trunk barely at all.
The output gives the game away: it is an index on an arbitrary scale, not a volume in cubic centimeters or a mass in grams, and the numbers are not comparable between brands. It also moves with hydration, with when you last ate, drank or exercised, and with skin temperature.
As a rough trend line, measured under identical conditions over months, it is not worthless. As a measurement of the thing it names, it is a model's guess. A tape measure costs less and tells you more.
What actually reduces it
An overall energy deficit is the lever. You do not get to choose which depot the body draws down, and no exercise selection changes that — the tissue that is worked is not the tissue that is spent. The encouraging part is that visceral fat tends to respond early. In imaging studies it typically falls proportionally faster than subcutaneous fat in the first phase of weight loss, which is why the waist often changes noticeably before the person in the mirror looks different.
- Aerobic activity. Walking, cycling, swimming — anything that accumulates volume. Meta-analyses of exercise trials find that moderate aerobic training reduces visceral fat even when body weight barely moves. That is not spot reduction; it is the effect of overall expenditure and improved insulin sensitivity, and it would happen the same way if you trained your legs.
- Resistance training. A smaller direct effect on visceral fat, but it preserves muscle during weight loss, and muscle is what keeps energy expenditure from falling further than it has to.
- Less alcohol. It supplies energy that almost nobody counts as food, and it is independently associated with fat accumulating in the liver.
- Sleep. Short sleep raises appetite and shifts the composition of weight lost toward muscle at the same deficit — the connection is covered in sleep and weight.
What does not work: crunches, planks held for the abdomen's sake, sauna belts, body wraps, vibrating massagers, and creams sold as fat-burning. Abdominal exercises strengthen the muscle wall, which sits between the visceral fat and the outside world; the fat is spent on general terms regardless. None of these has a mechanism by which fat behind the abdominal wall could be moved locally, and the ones that produce a same-day change in the tape reading are moving water, not fat.
When it becomes a doctor's question
If your waist is above the threshold that applies to you, the sensible next step is not a diet found online but a basic workup: fasting glucose or HbA1c, a lipid panel, blood pressure, and liver enzymes. Decisions are then made on the results rather than on the shape of the abdomen.
Some patterns should skip the tape measure entirely. A waist that grows quickly without weight gain, or that comes with swelling in the legs, breathlessness, or abdominal pain, is not a question about fat — fluid accumulation and other conditions look similar from the outside and are treated completely differently. That is a same-week appointment, not a diet.
Two other cases. In pregnancy, waist circumference and waist-to-height ratio do not apply and intentional weight loss is not the goal; nutrition decisions belong with the clinician managing the pregnancy. And if you take medication for diabetes or high blood pressure, losing weight changes what you need — insulin and sulfonylurea doses in particular may have to come down to avoid hypoglycemia as insulin sensitivity improves. Those adjustments are made by the prescriber, not by you, and they are a reason to tell your doctor that you are starting rather than to wait until something goes wrong.
