Creatine and the brain: what has been measured outside the gym

Creatine is one of the few sports supplements with a solid evidence base for strength performance. In recent years, it has also been marketed as "brain fuel." Data here is much scarcer and works differently: the effect is not seen in everyone, but rather in those whose baseline levels are depleted for some reason.

Why the brain is part of the conversation at all

Creatine phosphate acts as a rapid ATP buffer—wherever energy is needed instantly. Muscle is not the only such tissue: the brain consumes about a fifth of the body's energy while accounting for about two percent of its mass. Hence the hypothesis: if you increase the brain's creatine stores, resistance to fatigue should increase.

Two reviews and a debate on methodology

A systematic review and meta-analysis of randomized trials on memory in healthy individuals described improvements in memory performance with creatine compared to a placebo, with more noticeable effects in older participants — Nutrition Reviews.

A more recent review on cognitive functions included 16 randomized trials and 492 participants aged 20.8 to 76.4 years. Significant effects were found for memory (standardized effect size 0.31), attention span, and information processing speed; all included studies used creatine monohydrate. The authors conclude that larger and more robust trials are needed. Review in Frontiers in Nutrition. This paper later received a correction and a published commentary with methodological objections—the field is currently at a stage where experts are debating the methodology rather than the magnitude of the effect.

The overall picture from both works: there is a signal, it is small, there are 492 people in total, and the duration of individual trials is measured in weeks.

Where the effect is more noticeable

A recurring detail: the difference from a placebo is more often seen where the initial state is worse. This includes older participants, people with sleep deprivation, vegetarians, and vegans.

The latter is explained by simple arithmetic. Creatine is obtained from meat and fish, and the body also synthesizes it itself. In a person who does not eat meat, intake is lower, and a supplement shifts the stores more significantly than in someone who eats meat daily. What is known about safety

Creatine monohydrate is one of the most studied supplements in sports science. The most common reported effect is weight gain in the first few weeks due to water retention in the muscles, usually around one to two kilograms. For someone watching their weight, this looks like a setback, even though fat mass has not changed.

A separate issue is creatinine in lab tests. Creatine breaks down into creatinine, so its level in the blood can rise during supplementation, and the lab report may show a figure that is interpreted as impaired kidney function. For those with kidney disease or those on long-term medication, this issue should be discussed with a doctor, not on a forum.

The condition that gets lost in translation

A lot has been known about muscles and creatine for a long time. About the brain—little, for a short time, and in small groups, and the effect, according to the data, depends on how depleted the stores were initially. This is not the same as saying it "improves cognitive functions": the phrasing that holds up to the data is more modest and contains a condition.
